To support and befriend older adults with dementia and/ or other mental and physical health problems who are under the Lewisham Older Adult Community Mental Health Team. The Recovery Enabler will offer support to allocated service users. Examples of input may include: -Visiting people at home to ‘befriend’ them, providing much needed face-to-face social contact. This may then lead to developing wider community social links. -Accompanying people into the community (e.g. to the shops or to a café) to facilitate social inclusion or to increase their confidence in doing these things alone -Supporting people to identify community groups that match their interests and accompanying to these groups in the first instance -Accompanying people to hospital or other visits (e.g., hairdresser / dentist) where the outcome will improve their Recovery. -Co-facilitating social inclusion/lifestyle groups running at the CMHT team base alongside other professionals (e.g. occupational therapist or support worker)
