Community Health Navigator Role as part of the Community Health Programme will help and advocate residents with improving their health and wellbeing by signposting them to other health or voluntary services and activities as well as undertake simple health checks. For example, blood pressure, weight and BMI Also to encourage and help residents where needed to use the NHS App to book GP appointments, order repeat prescriptions etc.

For example, blood pressure, weight and BMI Also to encourage and help residents where needed to use the NHS App to book GP appointments, order repeat prescriptions etc. • Undertake training as specified to become a Community Health Navigator volunteer. • Support with coordination of local community activities • Promote and communicate monthly activities and Health initiatives. • Greet residents and discuss their individual health needs, logging with permission data and information. • Undertake key health indicators, such as Blood pressure, weight etc. (Training Provided) • Identify with residents, key engagement activities to address their personal health concerns. • Signpost and refer residents where possible to suitable activities and partnerships for support. • Encourage and help those residents who need help, with installing and registering on to the NHS App so they can book GP appointments, order repeat prescriptions, access their health records etc through the App. • Record all data of activities and communications. • Support with the collation of Case studies and changes within your local area Any questions contact Lisha Varghese at LishaVarghese@vcsutton.org.uk or 020 8661 5900.

• You have to 18years and over. • Have an interest in the local community and people's health. • Have a passion for helping other people. • To have a positive, non-judgemental attitude towards people with different cultural and health backgrounds • Ability to work with people on a one-to-one basis. • Enthusiastic, reliable, and encouraging • Patient and a warm and friendly attitude • Strong communication and interpersonal skills • Ability to keep sensitive personal information confidential. • Committed to equal opportunities. • To be able to complete meeting record sheets
